The rental amount is based on a maximum of _____people occupying the cottage. We want you to have a great time while you are staying at our Put-In-Bay cottage. We hope you find our cottage comfortable and relaxing. We attempt to maintain it in the best possible condition for our customers. However, we have several rules designed to protect our property from damage as well as protect our ability to rent our cottage. We have Conditional Use permits on our cottage that have been granted by the Put-In-Bay Zoning Commission. These permits are reviewed on an annual basis and can be revoked at any time by the Zoning Commission if neighbors complain or zoning rules are violated. Violation of the following rules can result in loss of rent and deposit and eviction from the property:
1.) No subletting to friends, children, etc. The person named in this agreement must reside at the home throughout the rental period.
2.) The occupancy limit must be maintained at all times. House parties, bachelor parties, beer kegs, fireworks are not allowed.
3.) No pets of any kind will be allowed in the cottage or on premises.
4.) Misrepresentation of the type of group and/or the ages of occupants. (Groups containing all males or mostly males will result in a minimum of $250 increase in rent amount stated above and a minimum deposit of $550) I have read the rules on this form as well as the rules on the rental agreement and I understand that I will be held responsible and charged for any damages occurring to the property that I am renting from “A Frame In The Woods” during the rental period stated on the agreement.

ARRIVAL AT COTTAGE INSTRUCTIONS
When you come in, breaker #3 is the hot water tank. Turn this on, it heats up quickly.
When you leave turn #3 breaker off.
If you choose to use the grill, remove cover, fold as to not blow away. Turn on gas valve on the tank. Follow instructions on the grill to light. When finished with grill please clean the grill surface, turn valve back to off, and cover.
If you are here on Sunday night, please put the BFI can up to the side of the road, or before you leave.
The cottage has a TV, radio and VCR. There are tapes in the TV cabinet. There is a basket of books with a variety of reading material. ?If it is cool, follow instructions on the heater. Make sure you turn it down before you leave.
Please make sure all windows are closed before you leave.
DO NOT lock bottom lock, key only for the dead bolt.

DEPARTURE INSTRUCTIONS Before Checking Out
Close and lock all windows and doors
Turn off gas grill and clean gas grill (including underneath grill) and cover
Wash and dry all dishes and place in cupboards ?Turn off Breaker #3 (hot water tank)
Dispose of all items in refrigerator. You can leave the beer!
Pick up all trash around the property including cigarette butts, beer cans, etc
Put all trash in trash can or in a trash bag next to trash can if full. If Sunday evening, put BFI can up to the side of the road.
Put all items back in their original place, including the comforters.
Unless otherwise instructed, turn OFF A/C and heat
Take all your items with you, but don’t take ours!
Please leave the house in the same condition you found it.
Do NOT lock bottom lock, key is only for deadbolt.
Put Keys back in the lockbox. Remember-You will be charged for anything more than a light cleaning.
RULES & CONDITIONS OF AGREEMENT
Lock box is attached to the bench at the front of the house. Code will be provided once contracted for rental.
Please bring your own sheets and towels. There are three full sized beds.
All cooking utensils, pots and pans are at the cottage.
No pets allowed
Motorcycles not permitted
Do not bring bicycles into the cottage
Be respectful of neighbors when playing radio or TV
No camping permitted on the grounds
Only the original renters are allowed to stay
Please be respectful of the cottage and the furnishings ( this is our home also) Leave all items that are in the cottage in the cottage
Although the cottage is cleaned before each rental, you are responsible to leave the cottage CLEAN. Trash out of the house, kitchen utensils clean and put away.
Bring any groceries you may need, although there is a grocery store on the island, with beer at State Min.
Last rule, have a great time and enjoy all that South Bass Island has to offer
